First-year Student Athlete

All first-year Chaminade University student athletes (freshman or transfers) must submit a pre-participation health form and a copy of their current health insurance card prior to arriving on campus. Once arriving on campus, pre-season physical examinations will be conducted at a date and time to be determined. Student athletes can download their form (below) and mail it along with their health insurance card (both front and back).

Returning Student Athletes

All returning Chaminade University Student Athletes must submit an Annual Health History Form, Insurance and Emergency Information Form, Assumption of Risk and Acknowledgement of Insurance Form prior to arriving on campus. Student athletes can also download the forms (below) and mail it along with their health insurance card capy (front and back).
